Dear Patients,

This letter is to inform you that effective November 30, 2025, Diare Dermatology will transition to The Hair NP and will no longer provide general dermatology medical services.

Our office will remain open and continue offering a wide range of aesthetic treatments, including:

• Hair and eyebrow restoration

• Injectables (Botox, fillers)

• Skin rejuvenation treatments

• Skin tag and DPN removal

• Laser hair removal

In addition, our online Shop will remain open for you to purchase your skincare products with the introduction of new hair care products.

Because of this change, I will no longer be able to serve as your dermatology provider after November 30, 2025. To ensure your medical care is not interrupted, I encourage you to establish care with another dermatology group you trust. We recommend reviewing your insurance directory to select a provider convenient to you.

If you would like your medical records transferred to a new provider, please complete our Medical Records Release Form and return it to us with your provider’s fax number. Record requests will be processed within 10–15 business days. Patients who prefer to pick up their records in person should notify our office.

After November 30, 2025, all medical records will continue to be securely maintained in accordance with New Jersey law. Records may be requested at any time by contacting our office. Please allow up to 25 days for processing.

Please note that state law permits a reasonable fee for copies: $1.00 per page for the first 100 pages, $0.25 per page thereafter, plus postage if mailed.

During this transition, I will continue to provide dermatology care for existing patients with established conditions. We will not be able to treat new conditions after this date. If you are an existing patient  with new insurance or have a new condition requring treatment, please consult with your new dermatology provider. Medication refill requests for patients seen within the last six months must be submitted before November 30, 2025.

It has been a true honor and privilege to care for you at Diare Dermatology. Your trust and support over the years have meant so much to me. I am deeply grateful to have been part of your health journey, and I look forward to continuing to serve you through The Hair NP’s aesthetic services.
